1). What experience do you have?
2). Describe your work history?
3). What financial planning designations do you hold?
4). What are your educational qualifications? Your college degree is in ?
5). What services do you offer?

6). What security registrations do you hold?  Are you restricted from selling certain products or investment vehicles because of the registrations you do not hold?
7). What financial planning continuing education requirement do you fulfill?
8). What is your approach to financial planning?
9). Will you be the only person working with me?
10). How will I pay for your services?

11). Could anyone besides me benefit from your recommendations?
12). Have you been publicly disciplined for any unlawful or unethical actions in your professional career ?
13). Are you a captive insurance agent?  Does your contract, with the insurance company, require you to produce a certain amount of insurance premium business per year with them?
14). Do you sell or recommend proprietary products?
15). Will you provide me with your disclosure document form ADV Part II or its state equivalent?
